Release of Information

Covenant HealthCare's Health Information Management department strives to provide extraordinary care by assisting you with your health information needs. 

For your convenience, authorization forms for the release of health information and health record amendments are available online. These forms must be submitted to our department with a copy of driver's license or picture identification, and can be filled out at our location, mailed, faxed, or scanned and sent via email. Please use our contact information below. Some exceptions may apply.

There may be a charge for medical records requested. Our fee schedule is as follows.

  • $0.48 per page up to 50 pages
  • $0.23 per page after 51 pages

Please note that our office is located on the first floor of the 900 Cooper building, which differs from our mailing address. To find our office, enter 900 Cooper through Entrance B, and turn to the left toward the sliding door. Continue through the sliding door and our department is the first door on the left-hand side.
