Advance Care Planning Facilitator Certification Course

We are pleased to announce that we will be offering an Advance Care Planning (ACP) Facilitator Certification class this fall. As in the past, we will be providing this training in collaboration with Respecting Choices® (RC), an internationally recognized, evidence-based system for person-centered decision-making that transforms the healthcare culture by honoring an individual’s goals and values.

Outcomes for participants include:

  • Verbalize strategies to identify intended populations and engage individuals and healthcare agents in ACP.
  • Demonstrate communication skills to promote person-centered ACP conversations.
  • Apply critical thinking to discern what matters most to individuals and use of person-centered decision-making framework to elicit preferences and decisions.
  • Create strategies to transfer individual goals, values, and preferences to a written plan.
  • Demonstrate beginning competency in facilitating First Steps ACP conversations through role-play activities.
  • Identify Michigan legal requirements for Advance Directives

Certification as a First Steps ACP Facilitator will be offered to all who complete the following:

  • Complete Respecting Choices Online ACP Facilitator Curriculum (modules 1–4 for First Steps) – Completed prior to class
  • Full participation in the Respecting Choices First Steps ACP Facilitator Certification course – In person one day event
  • Role-play or shadowing validation to demonstrate ACP skills– Completed following class
