Medical Records Requests

At Covenant HealthCare, we are committed to making access to your medical information simple, secure, and efficient. To better serve you, we partner with HealthMark Group, a trusted partner in health information management, to process requests for medical records.

This partnership allows us to streamline the request process and deliver your records quickly, so you can focus on your health.

How to Request Your Medical Records

To get started, use the secure online request system: https://requestmanager.healthmark-group.com

Once you enter your email address, you will receive a secure email from HealthMark. Simply follow the link provided—no username or password required. From there, click “Submit Request” and follow the prompts to complete your request.

You will be notified by email as soon as your records are ready.

How You’ll Receive Your Records

HealthMark’s Request Manager uses secure email verification and encrypted links to deliver your records safely and efficiently. Your records will be sent directly to your email inbox, making access quick and convenient, without the need to create or remember additional login credentials.

Processing Time

Most medical record requests are processed within 8 business hours. In most cases, you can expect to receive your records electronically within one to two business days after submitting your request.

Requesting an Addendum to Your Medical Record

If you believe your medical record needs to be updated or corrected, you may request an addendum for review. To do so, please submit the form linked below along with a copy of your driver’s license or other valid photo identification. Forms can be completed in person at our office, or returned by mail, fax, or secure email using the contact information provided below. Please note that some exceptions may apply based on the nature of the request.
