Appointment and Privileges

Requests for Medical Staff appointment and privileges at Covenant HealthCare can be made by contacting the Medical Staff Office at 989.583.6595. Covenant HealthCare utilizes an on-line web application system called MD APP. An email address is required for application purposes. Links for application are active for only 24 hours. Once the link is activated, provider must complete application within 14 days. Failure to access link within 24 hours will require request to the Medical Staff Office for a new link (this is only granted one time)

Application Process

Applicants will receive an auto-generated email invitation from service@mdapp.com with two links enclosed. The first is to start the application and the second link is to create a password for the MD App system. Applicants will need to create a password FIRST, then will be directed to the application.

Again, the initial link is active for only 24 hours. You will need to create an account/password to access the application. Please retain this information for continued access to the application system.

Applications need to be submitted within 14 days of receipt of the email.

Instructions on how to complete the application are included with each section. Please always complete each section as fully as possible, even if the areas are not marked required. Subsequent reminders and/or communications will also come from the MD APP system email address. Please add this email address to your contact list to avoid future communication being inadvertently directed to your Junk/Spam folder.

There may also be included with your electronic application other documents that need to be downloaded, signed and then re-uploaded. These will be under the “Files” section of your application email.
